Question

PCOS

Recently a global conference for treatment of infertility in women with PCOS (Polycystic Ovarian Syndrome) was held in Greece.  The treatment should be tailored to each patient's individual circumstances and expectations.

-before medical treatment, patient's lifestyle changes should include daily exercise, weight loss, stop smoking and reduced alcohol consumption.

-nutritional counseling with a dietician is helpful and motivational.

Since PCOS patients have a greater number of potential eggs to stimulate each month, they are at greater risk of multiple pregnancy.

Treatment options for infertiltiy:

1.  Clomiphen Citrate (CC) for ovulation induction (oral tablets)*

2.  Gonadotropins (subcutaneous daily injections for several days)*

3.  In Vitro Fertilization.

4.  Laparoscopic ovarian drilling to decrease ovarian cysts.

5.  For PCOS patients with insulin resistance, Metformin may be added to the above treatment options for potential increased live birth rate and reduced multiple pregnancy risk.

*The cumulative singleton live birth rate for clomiphene citrate and gonadotropins is 72%.
